We want to recognise you for all of the continuous hard work you do to keep your patients happy and healthy.
That's why we are introducing the One You Lincolnshire Affiliate Award scheme for surgeries.
Each quarter, two surgeries will be chosen from a selection of practices that we deem to be 'high performing' in terms of engagement and referrals.
Who it's for:
Surgeries who show a strong commitment by going the extra mile, providing their patients with care to support their health and wellbeing and fight health inequalities in Lincolnshire.
We'll be looking for surgeries that have:
- An appointed One You Champion.
- A sustained level of referrals across all One You pathways, relative to patient list size.
- Undertaken targeted messaging via text or mail to specific patient groups with our support.
- Attended a clinical meeting within the past 12months to discuss programmes and patient outcomes.
- A good turn around time for stop smoking prescriptions and exercise referral sign-offs.
- Engaged with us across social media platforms.
And one of the following:
- Hosted a clinic.
- Set up a One You display board.
- Attended a One You webinar.
- Provided One You with a case study.
What you receive:
- A shiny plaque to sit pride and place in your waiting area.
- A special visit from a Partnership Officer and our Head of Service.
- Free media coverage for the fantastic work you do.
