This programme offers older adults structured physical activity sessions. These are designed to improve and develop strength and mobility. We hope this will empower them to live healthy lives and reduce the risk of falls.

This programme offers older adults structured physical activity sessions. These are designed to improve and develop strength and mobility. We hope this will empower them to live healthy lives and reduce the risk of falls.

What we offer:

  • A 24-week programme delivered in venues across Lincolnshire.  
  • 1-2-1 tailored support, outside the weekly sessions, from the Strength and Balance team.
  • Resources available to motivate and guide you to make changes, including strength and balance exercises and healthy lifestyle advice.

  • A discovery conversation with a member of our team, explaining about the programme and understanding your starting point.  
  • 24 weekly strength and balance activity session in a venue close to you.  
  • After each weekly session there will be a chance to chat with fellow participants over a hot drink.  
  • Resources to guide you to complete exercises in the comfort of your own home.
  • A final appointment with our team to review your success in the programme and a signposting opportunity to community physical activity of your liking.

WHO WE HELP

  • You must be a Lincolnshire resident, or be registered with a Lincolnshire GP
  • You must be aged 65 and over
  • Your Health Care Professional must deem you at risk of falling
  • You must be able to walk independently. (with or without a walking aid)
  • Healthcare Professionals must refer you

How it works

First appointment

We will contact you to arrange a suitable time for your first appointment. During this 30 min appointment you will discuss any concerns you have about quitting, the benefits you are hoping to get from stopping smoking and how the appointments and NRT work.

sending & receiving nrt

Nicotine replacement therapy comes in a variety of forms. The Stop Smoking practitioner will discuss with you the type of NRT which might work best for you. It will be posted to your address.

SETTING A QUIT DATE & QUITTING

Your Practitioner will help you plan and prepare for your quit date, taking into account any concerns or barriers you may feel towards quitting.

WEEKLY APPOINTMENTS

The course runs for a total of 12 weeks, and you will be able to receive weekly or bi-weekly appointments depending on the level of support you need. These appointments are roughly about 15 minutes in length where the practitioner will review your quit product (e.g. NRT) and discuss any concerns or struggles you might have had that week.

FOUR WEEK QUITTER STATUS

This is a great goal to aim for during Stoptober. Your practitioner will help adjust any NRT requirements you may have at this point.

TWELVE WEEK QUITTER STATUS

Congratulations you have reached the end of the course and are a fully fledged non-smoker.

SIX / TWELVE MONTH FOLLOW-UP

We will keep in touch to offer any further support that you might need through our other pathways. And if a relapse does occur, remember you can always come back and try again.

How it works

Our team receive a referral for you from a Health Care Professional / GP.

We'll aim to be in touch with you within 5 days to discuss what will work for you needs and goals. As well as this, we will run through the programme available to you and hopefully help you to get signed up!

The team will send you details of when your programme starts, and you’ll begin your sessions with us!

After the 24-week strength and balance progressive programme you will have a final appointment with our team to discuss next steps and see how amazing you have done on the programme.
